Participating Carriers
Out of Network Services
If your health insurance plan is one in which we do not participate, we will be glad to see if you have out of network benefits.
If you utilize your out of network benefits you will be responsible for all charges, payable at the time of service. As a courtesy we will send claims to the insurance company.
Referral Statement
If your insurance plan requires a referral from your primary car physician, it is your responsibility to have the referral at the time of appointment. If you do not have a referral at the time of appointment, we will ask you to sign a form stating that you will be responsible for charges if a referral is not obtained.
